Thank you for considering Bloom PT for your physical therapy needs. Please take note of the following before booking:

  • We specialize in treatment of vestibular disorders, dizziness/ vertigo, balance issues, concussion, and neurological disorders such as FND (functional neurological disorder), Parkinson’s Disease, or Stroke. If you are seeking services for another issue, please reach out via phone (585-502-9531) or email (hello@bloom-pt.com) to determine if we will meet your needs.

  • We are currently OUT of network with *most insurance providers. This means that we do not submit claims to insurance companies and payment is due in full at time of service. We will provide you with a detailed receipt if requested, so that you can submit it to your insurance company for possible reimbursement. We require a card on file to book online, but it will NOT be charged until services have actually been provided.

  • We are currently IN network and accepting Aetna, MVP, and Medicare Part B insurance plans.

  • We are a mobile practice servicing the Rochester, NY area. This means that we do not have a physical location. Our therapist travels to your home. We are only able to serve the Rochester, NY and close surrounding areas at this time. Please contact the office if you have questions about our service area. Telehealth services available throughout NY state.

Rachel Gulvin
(She/Her/Hers) PT, DPT

My name is Rachel Gulvin. I’m a licensed physical therapist who is passionate about working with people with vestibular and neurological disorders, particularly Parkinson’s Disease, FND, and movement disorders. I earned my Doctor of Physical Therapy degree from Ithaca College. Over the years, I’ve completed extensive training, continuing education, and worked in several outpatient and hospital-based settings, where I’ve gained invaluable experience working with these populations. I founded Bloom Physical Therapy in 2023 to serve people with vestibular or neurological disorders who are looking to regain function, return to meaningful activities, or rediscover joy in their lives. Bloom Physical Therapy currently offers at-home and telehealth services to those in the Rochester, NY area.
