Thank you for considering Bloom PT for your physical therapy needs. Please take note of the following before booking:

  • We specialize in treatment of vestibular disorders, dizziness/ vertigo, balance issues, falls, older adults, and neurological disorders (such as Parkinson’s Disease, Stroke, Functional Neurological Disorder, etc.). If you are seeking services for another issue, please reach out via phone (585-502-9531) or email (hello@bloom-pt.com) to determine if we will meet your needs.

  • We are currently IN network and accepting MVP Healthcare, Excellus BlueCross BlueShield, and Medicare Part B insurance plans.

  • We are currently OUT of network with all other insurance providers. This means that we do not submit claims to these insurance companies and payment is due in full at time of service. We will provide you with a detailed receipt upon request, so that you can submit it to your insurance company for possible reimbursement.

  • We offer both In-Clinic appointments in our Penfield, NY office and Mobile/ In-Home appointments throughout the Rochester, NY and close surrounding areas (please reach out to the office if you aren’t sure if we travel to your area). Please double check to make sure you are booking in the correct location.

Physical Therapy

We offer specialized physical therapy services for those with vestibular disorders, dizziness/ vertigo, balance issues, concussion, or movement disorders.

Rachel Gulvin
(She/Her/Hers) PT, DPT

My name is Rachel Gulvin. I’m a licensed physical therapist who is passionate about working with people with vestibular and neurological disorders, particularly those with dizziness/ balance issues, Parkinson’s Disease, and movement disorders. I earned my Doctor of Physical Therapy degree from Ithaca College. Over the years, I’ve completed extensive training, continuing education, and worked in several outpatient and hospital-based settings, where I’ve gained invaluable experience working with these populations. I founded Bloom Physical Therapy in 2023 to serve people with vestibular or neurological disorders who are looking to regain function, return to meaningful activities, or rediscover joy in their lives. Bloom Physical Therapy currently offers both in-clinic physical therapy appointments in our Penfield, NY office, and in-home physical therapy visits to those in the Rochester, NY area.
